Humans play a crucial role in socio-technical systems. With the rapid advancement of science and technology, human involvement has become a dominant factor in the occurrence of various incidents in organizations. Accordingly, improving human reliability serves as an effective strategy to reduce human errors. Identifying performance-shaping factors is a critical issue in determining the probability of human error and, ultimately, human reliability. This study, employing a systematic review approach, aims to investigate the role of human reliability analysis in organizations and identify the performance-shaping factors influencing the occurrence of human errors. The findings of this research emphasize the importance of focusing on human reliability and its significant impact on enhancing the performance of production and service organizations. It also highlights the necessity of utilizing modern techniques, such as simulation methods, for its evaluation. This study provides managers and policymakers with insights into common performance-shaping factors, elucidates the importance of continuous monitoring and analysis of human reliability in improving safety management and reducing human errors, and lays the foundation for planning and formulating appropriate strategies to mitigate occupational risks.
